The following forms must be submitted simultaneously for an application to be considered:

1. Application Form
2. Developmental History
3. Release Form
4. Educational and Psychological Testing (within one year)
5. Any other evaluations which will help us understand your child.
6. Projectives Testing for applicants to grades 6-12
7. $50.00 Application fee

The following may be submitted separately:

• Admissions Questionnaire may be returned directly to the school by the person filling out the form. This form is completed by the child’s teacher, and anyone else who works with the child (i.e. speech and language therapist, occupational therapist, resource room teacher, guidance counselor, school psychologist)
